Registrations by fax require payment by credit card (Visa, MasterCard, or American Express). Registration is not considered complete until payment has been received. Online Registration
Complete your registration form online at www.shsmd.org, and submit your credit card payment over our secure server.
On-Site Registration
On-site registration will be available. Conference registration opens at 8:00 a.m. on Sunday, September 12, and runs through 10:00 a.m. on Wednesday, September 15. To register on site, you may use a major credit card (Visa, MasterCard, or American Express) or bring a personal or company check or money order. Please note that the availability of materials for late or on-site registrants cannot be guaranteed.
Conference registrations with an outstanding balance may be paid on site by credit card, personal or company check, or money order. If your company check has not been received prior to the beginning of the conference, a valid credit card number must be given in order to complete your registration.
On-Site Check-In
Your official badge, issued at the conference, will be required for admission to social functions, educational sessions, and the exhibit hall. Registration and conference materials will be available during registration hours at the Hyatt. All registrants with unpaid balances will be asked to pay on site with a check or credit card before being admitted to the educational sessions or the exhibit hall.

Full-Conference Registration
When you register for the full conference, you receive admission to the general sessions, concurrent sessions, Exhibit Hall, welcome reception, networking events, continental breakfast daily, and refreshment breaks, as well as lunch on both Monday and Tuesday. Full-conference registration also includes electronic access to the handouts for all concurrent sessions. Handout material will be available online for download before the conference so you can better prepare for the programs you plan on attending. Nonmember registration includes a full-year of SHSMD membership.
While SHSMD strives to meet the dietary needs of our attendees, the Hyatt and surrounding area has an extensive variety of restaurants to meet your individual dietary preferences.
Single-Day Registration
If you cannot attend the full conference, we encourage you to consider single-day registration and attend only one or two days of the meeting. Registration fees include all scheduled programs and events for that day.
Online Conference Recordings
All of this year’s concurrent sessions will be professionally recorded and synched with the appropriate PowerPoint presentation online. Conference attendees may purchase this downloadable version of the sessions prior to the conference for only $149. To order at this special rate, please select the Best Value registratino category or chose the Online Conference Recordings option on the SHSMD Registration Form.
Guest Registration
SHSMD considers a spouse, partner, or family member who is at least 18 years old and attending the conference to be a registered guest. Guests are allowed to enter the Exhibit Hall and attend all conference food functions. Guest badges will show first and last name only, and guests are not permitted to conduct business with SHSMD attendees or exhibitors.
Confirmation Policy
All paid registrations will be confirmed via e-mail or fax (if an e-mail address is not provided). Please call SHSMD at 312.422.3888 if you do not receive a confirmation notice. AHA reserves the right to cancel or reschedule programs and to close registration when programs are oversubscribed. For these reasons, registrants are advised against purchasing non-refundable airline tickets.
Cancellation Policy
SHSMD reserves the right to cancel or reschedule the Annual Conference or individual sessions due to unforeseen circumstances. Registration fees, less a $150 cancellation fee, are refundable if written notice is received by September 3, 2010. Cancellations of unpaid registrations also are subject to the $150 cancellation fee. After September 3, 2010, all fees/credits toward the conference are forfeited. SHSMD cannot be held liable for non-refundable airline tickets.
Please direct written cancellation requests to SHSMD, Attention: Sharon Warren, 155 N. Wacker Drive, Suite 400, Chicago, IL 60606; alternatively, fax your cancellation request to 312.278.0883, or e-mail it to shsmd@aha.org. Cancellations will be acknowledged within five business days. If you do not receive an acknowledgment that we have received your cancellation, please call 312.422.3888. (SHSMD will not refund conference fees for cancellations that have not been acknowledged in writing by September 6, 2010. Please be sure to confirm your cancellation in order to receive a refund.) Please note that all refunds will be made to the payer only. No exceptions. You are encouraged to send a substitute if you cannot attend.
Substitutions
Registrants unable to attend the conference may send a substitute. If the substitute is not an SHSMD member, the nonmember cost differential must be paid in advance or on site. Registrants unable to attend the conference may also request that the registration fee be applied to another SHSMD program within a 12-month period. Cancellations of unpaid registrations and transfers to other SHSMD programs are subject to a $150 cancellation fee.
Restriction
By their registration, program participants agree to refrain from marketing products or services during the course of this conference.

Recordings
This program will be electronically recorded by the American Hospital Association. By attendance and/or participation in discussions, registrants agree that the American Hospital Association may electronically copy, record, and distribute registrants’ attendance at and involvement in the program discussion and question-and-answer periods. No individual or entity other than the American Hospital Association may electronically record any portion of this program for any purpose without the prior written consent of the American Hospital Association.
NOTICE
SHSMD reserves the right to cancel or reschedule a session because of an insufficient number of registrants or other unforeseen circumstances and cannot be held liable for any travel expenses incurred.
